Membership Rules
Membership is available to anyone 16 years of age or older. Valid picture I.D. required.
The membership fee is for 12 months from the member’s join date and is renewable
Membership cards are NOT TRANSFERABLE.
Only the individual to whom the card is issued may use the card.
Do not destroy the membership card; new cards are not issued upon renewal.
Membership cards must be presented when checking in at the event.
Membership cards have priority seating in selected card holder’s membership seating area.
Membership card holders may attend all Pre & Post events & parties.
All Members will receive an Annual Tax-Deduction Receipt from MC Healthcare Inc.
Lost or Stolen Cards
Report lost or stolen cards to MCH Operation Manager Charla at charla@mchealthcareinc.org
There is a $10.00 fee for replacement.
Privacy
MC Healthcare Inc. respects your right to privacy. If you do not want us to share your name and address with outside companies, please inform our customer service personnel.
Payment
We will ask for your identification and your telephone number if it is not imprinted on the check.
Should the bank return a check; the member is responsible for the face amount of the check and a return check service fee.
Membership may also be revoked. No second payee or payroll checks accepted.
We accept MasterCard, Visa, Discover, Amex and Debit Cards.
General Rules
MCHealthcare Inc will not be held liable for injuries, and /or death at any event member attends.
Members will be required to follow all rules & regulations at each event.
Anyone entering the event may be searched for weapons of mass distruction. This includes bags, backpacks or bags of goods or property.
MC Healthcare Inc reserves the right to inspect any container, backpack, stroller, etc, upon entering and leaving the event.
Shirts and shoes are required. Pets are not allowed. Picture and video taking is not allowed.
Cancelation Policy
7 Business Days from your donation payment date "Including Sunday - Saturday" you receive a full refund of your membership payment. Any cancelations after 7 Days will not be refunded.
